  • A written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees.

We place an emphasis on education, and therefore we encourage questions from both the new patient and the parent. Completing and submitting the forms beforehand will help save time during your visit. We want to make your first visit, and all your subsequent visits, as comfortable and stress-free as possible.
