Requirements to Apply for The William R. Parks Memorial Scholarship:

1. Applicant must be a senior in high school or a current college student.
2. Applicant must be a current or former patient of Parks Orthodontics.
3. Applicant must fill out the simple application form below.

The deadline for applications is May 1, 2024. Any applications submitted after this date will not be considered for the scholarship.
